We have a responsibility to look into the circumstances when a child dies, as set out in the guidance on working together to safeguard children.

We must arrange for:

• a rapid response by a group of key professionals who come together for the purpose of enquiring into and evaluating each unexpected death of a child
• an overview of all child deaths in the LSCP area

Reporting and Managing a Child Death

You can notify us of the death of a child usually resident in Medway, regardless of their location of death. You must complete Form A within 24 hours.
